ABSTRACT

Objective To explore the effect of establishment of green nursing channel for emergency patients with severe acute traumatic shock on quality of nursing care. Methods Fifth-six patients with severe acute traumatic shock who received nursing by green nursing channel admitted to Department of Adult Emergency of Second Affiliated Hospital of Wenzhou Medical University (Yuying Children's Hospital) from October 2016 to November 2017 were assigned in the research group, and at the same time, 42 patients with the same disease adopted conventional nursing mode from March 2015 to September 2016 were in the control group. The emergency treatment time, special inspection time, emergency to the operating room time, admission to operation time of patients, the incidences of the asphyxia and multiple organ dysfunction syndromes (MODS), and success rate of rescue in the two groups were recorded; the degree of patients satisfaction were observed. Results The emergency room treatment time (minutes: 13.51±3.62 vs. 20.84±3.96), special inspection time (minutes: 16.30±4.05 vs. 23.71±4.16) and admission to operation time (minutes:42.61±7.98 vs. 57.83±7.46) in the study group were significantly shorter (all P < 0.05), the incidences of asphyxia and MODS in the study group were obviously lower than those in the control group [asphyxia: 1.79% (1/56) vs. 11.90% (5/42), MODS: 3.57% (2/56) vs. 16.67% (7/42), both P < 0.05], and the success rate of rescue in the study group was significantly higher than that in the control group [100.00% (56/56) vs. 92.86% (39/42), P < 0.05]; the degree of patients satisfaction in the study group was obviously higher than that in the control group [96.43% (54/56) vs. 83.3% (35/42)], and the differences between the two groups were statistically significant (all P < 0.05). Conclusion The establishment of a green nursing channel for emergency patients with severe acute traumatic shock is helpful to improve the emergency working efficiency, shorten the time for nursing emergency response, and its application effect is good.

ABSTRACT

Objective:To compare the therapeutic and adverse effects of chemotherapeutic regimen based on three drugs (taxol+carboplatin/cisplatin+etoposide) and two drugs (carboplatin/cisplatin+etoposide) on the combined small cell lung cancer (CSCLC). Methods:A retrospective study was conducted based on the data of 62 CSCLC patients who were admitted to and treated at Tianjin Medical University Cancer Institute and Hospital between July 2000 and April 2013. Of the 62 patients, 19 received the three-drug regi-men and 43 received the two-drug regimen. All patients received at least two cycles of chemotherapy and completed follow-up proce-dures. For each patient, the therapeutic efficacy was evaluated every two cycles, and toxicity was evaluated every cycle. Results:The response rates between the three-drug and two-drug groups were statistically significant (90%vs. 53%, P=0.033). However, no statisti-cal differences were observed in the disease control rate between the two groups (100% vs. 86%, P=0.212). The three-drug regimen could induce a better median progression-free survival compared with the two-drug regimen, but with no statistical significance (10.5%vs. 9.8%, P=0.484). Similarly, no statistical differences were noted in the median overall survival between the three-drug and two-drug groups (24.0%vs. 17.5%, P=0.457). The incidence rates of grade IV bone marrow depression and the termination of the original regi-men owing to severe adverse reactions were both significantly higher in the three-drug group than in the two-drug group (26.3% vs. 7.0%, P=0.036;31.6%vs. 14.7%, P=0.004). Conclusion:The two-drug regimen had almost the same survival rate and lower toxicity compared with the three-drug regimen. When using the TEP/TCE regimen, a close attention should be focused on its adverse reactions. The findings of this work showed that the two-agent regimen should be one of the standard treatments for CSCLC.

ABSTRACT

<p><b>OBJECTIVE</b>This study was designed to investigate the correlation factors for occurrence and progression-free survival of patients with cavitating lung cancer.</p><p><b>METHODS</b>We collected the clinical data of 947 lung cancer patients. Tumor cavitation was observed in 51 patients at baseline and in 23 patients after treatment, while was not discovered in other 873 patients. Multifactor logistic regression was performed to analyze the correlation factors for occurrence. The independent predictors of PFS were analyzed with Cox proportional regression. Survival curves were constructed with the Kaplan-Meier product limit method and compared using the log-rank test.</p><p><b>RESULTS</b>In the 947 cases, the proportion of cases with baseline cavitation was 5.4% and the incidence of cavitation after treatment was 2.6%. Multivariate logistic regression analysis revealed that the occurrence of baseline cavitation is related to age, history of diabetes, history of drinking, pathologic types, tumor location, tumor diameter and distant metastasis (P < 0.05). Multifactor logistic regression analysis revealed that the occurrence of post-therapeutic cavitation is related to sex, pathologic types and tumor diameter (P < 0.05).The median PFS of patients with baseline cavitation (7.3 months) was significantly longer than the cases without it (5.2 months) (P = 0.002). While there was no significant difference between the median PFS of patients with post-therapeutic cavitation and patients without it (5.1 months vs. 5.3 months, P = 0.060). Cox proportional regression analysis revealed that cyfra21-1 is related to PFS of patients with baseline cavitaion (P < 0.05) and smoking history is related to PFS of patients with post-therapeutic cavitaion (P < 0.05).</p><p><b>CONCLUSIONS</b>Patients with baseline and post-therapeutic cavitation present different clinical features and progression-free survivals. The PFS of patients with baseline cavitation is longer than that of the cases without it. On the contrary, PFS of patients with post-therapeutic cavitation is shorter than the patients without it.</p>

ABSTRACT

Objective To study the relationship between the change of T lymphocyte subsets and NK cells in esophageal cancer patients and radiotherapy effect.Methods The levels of T lymphocyte subsets and NK cells were detected by flow cytometry in 56 cases with histologically confirmed esophageal cancer treated with radiotherapy and contrasted to the healthy people.Results In patients with esophageal cancer,peripheral blood T cells,Th cells,Th / Ts were decreased significantly compared with the control group [(58.3±5.2) % vs (65.8±7.2) %,(28.7±5.0) % vs (38.1±7.7) %,(1.0±0.3) vs (1.6+2.7),all P < 0.05],while the Ts cells were significantly increased [(28.8±5.3) % vs (25.4±5.7) %,P < 0.05].There was no significant difference between peripheral blood T lymphocyte subsets,Th/Ts ratio change and patient age,sex,tumor staging,histological differentiation and pathological lesions.After radiotherapy,the levels of peripheral blood T cells,Th cells,Th/Ts cell ratio and NK cells in esophageal cancer patients were increased [(66.9±4.5) % vs (59.4±4.9) %,(40.6±5.6) % vs (29.1±4.2) %,(1.6+0.5) % vs (1.0±0.4) %,(16.2±3.9) % vs (14.6±3.2) %,all P < 0.05],while the Ts cells decreased [ (25.4±3.6) % vs (28.4±5.7) %,P < 0.05].The increasing degree of peripheral blood T cells,Th cells were closely related to the lesion progress,the difference was significant (both P < 0.05).Conclusion Cellular immune function in patients with esophageal cancer is low.Detection of T lymphocyte subsets,NK cells can be used for immune monitoring of patients with esophageal cancer.

ABSTRACT

Tei-Index is a useful echocardiographic parameter for evaluating global cardiac function. Conventionally, a value and b value, which the calculation of Tei index is based on, are measured in different cardiac cycle. Therefore, accuracy of Tei index is damaged by cycle dependent variation, especially in patients with arrhythmia. The newly established Dual Doppler modality allows a synchronous measurement of both a value and b value. This study was aimed to explore the value of Tei index measured by Dual Doppler in cardiac function estimation in patients with hypertension. One hundred and forty patients with hypertension were recuited and divided into three groups according to cardiac function. The a value and b value were generated from the transmitral and transaortic flow spectrum of the same cardiac cyle by Dual Doppler modality. The results showed that Tei index was significantly higher in the cardiac dysfunction group than in the control group. There was a high correlation between Tei index and NYHA cardiac performance. Tei index measured by Dual Doppler modality is a highly reproducible parameter with high sensitivity and specificity for cardiac function estimation, even for patients with arrthymia.

ABSTRACT

BACKGROUND:Poly(D,L-lactide-co-glycolide)(PLGA)may produce lactic acid and glycolic acid when PLGA degrades,thus leading to the acid accumulation and inducing the inflammatory reaction locally.OBJECTIVE:To investigate the effect of lysine,histidine and arginine on regulating the acid accumulation of PLGA copolymer during the degradation.DESIGN:Repeated measurement and experiment.TIME AND SETTING:The experiments were performed in the College of Bioengineering.Chongqing University from July 2006 to August 2007.MATERIALS:PILGA(80:20)was produced by Sigma(USA);Lysine,histidine and arginine(purlty>99%)were purchased from Sigma(USA);Chitosan(deacetylation degree:85%)was purchased from Chengdu Kelong Chemical Reagent Factory;Algin(viscosity:1.05-1.15)was purchased from Tianiin Damao Chemical Reagent Factory.METHODS:Lysine,histidine and arginine were added into PLGA with the proportion of 5%(w/v)and 10%(w/v)respectively.The resultant film sample was put into a bottle with tri-distilled-water for 2-month degradation at 37℃.The pH value of degradation solution was detected by pH meter;Each film sample was taken out and lyophilized 12 hours to get its dry weight and calculate mass loss ratio.Each variety of the solution was sampled three specimens,the average pH value,average initial weight and average finial weight of these three specimens were taken as the indices at each sampling time point,respectively.Accordingly,the regulation effect of basic amino acid was comparexl with that of algin,chitosan and NaHCO3.MAIN OUTCOME MEASURES:The changes of pH value of degradation solution;the mass loss ratio of the composite.RESULTS:Each basic additive could relieve the acid accumulation,among them,NaHCO3 was the strongest,while algin and chitosan showed a lowest capacity,basic amino acid was moderate;The suitable regulation effect could be achieved at a level of 5%lysine.CONCLUSION:Basic amino acid can effectively regulate the acid accumulation after PLGA degrades,and the optimal concentration of lysine is 5%.

ABSTRACT

Objective To explore the natural forgetters' episode memory and personality characteristics.Methods Situational Self-Awareness Scale(SSAS),Russell Loneliness Scale and Zimbardo Time Perspective Inventory(ZTPI)are used upon 127 natural forgetters and subjects of control group.in which 90 subjects participate tions,natural forgetters performance worse than subjects of control group(t=1.76 one-tail,P<0.05),but subjects getters score lower in the SSAS and the future subscale of ZTPI(t1=1.73,P<0.05 one-tail;t 2=2.70,P<0.05),and tend to endorse in the present fatalistic subscale of ZTPI(t=2.19,P<0.05),but don't feel more lonely.Conclusion Natural forgetters episodic memory is selectively micro-injured under the enactment conditions,which is different from the aging impairment mode of selective source memory,and they have some special personality characteristics.
